Proviso Township Handyman Client Agreement

All appointments are scheduled via phone and are 45 MINUTES OR LESS. You are allowed one job per scheduled appointment. For multiple jobs, schedule additional appointments.

Proof of ownership will be reviewed through the Cook County Assessor’s office.

Applicants under 60 years old must provide proof of disability from Social Security or Veterans Administration.

Client is expected to be at home at the time of their appointment. Appointments may be subject to cancellation or rescheduling if the Client is NOT at home after 15 minutes of their scheduled appointment.

Clients must provide a minimum of 24 hours’ notice for any cancellation or rescheduling of an appointment. Repeat cancellations or no-shows without proper notice may lead to suspension and/or termination of services.

Client agrees to provide ACCURATE PARTS. The Client agrees to follow any instructions provided by the Handyman. The Client agrees to ALWAYS maintain a respectful and cooperative demeanor with the Handyman and office staff. It is the Client's responsibility to ensure contact information is current and accurate.

The fee for each appointment is $5.00. Payment is due at the time of service. Acceptable payment: CASH OR PERSONAL CHECK ONLY.

Proviso Handyman Referrals: It is important to understand that while we may provide referrals, these entities are separate and NOT part of Proviso Township. The contracting of services from a person or business referred by Proviso Township in no way constitutes any form of contract, binding or unbinding, between Proviso Township and any other parties involved. Please contact the service providers for information and be aware that the resident or homeowner is responsible for all payments and obligations to these service providers. Proviso Township is not responsible or liable for any damages, injury or loss resulting from the use of the services of a person or business referred by Proviso Township. The resident or homeowner accepts all liability from any damages, injury or loss caused by any person or business referred by Proviso Township.
